Vial of Life Program

Peacham's 1st Responders and EMTs Helping Neighbors with the Vial of Life

The Vial of Life program is an easy way to ensure that EMTs will know about your medical history and current medications at a time of emergency while keeping your information absolutely safe and secure.

The Peacham Volunteer Fire Department urges you to complete a "Vial of Life" Medical Information Sheet and keep it in your home in case of a medical emergency. Taking a few moments to do this could save precious minutes in a medical emergency. Simply complete one sheet of information per member of your household. It is very important to be as thorough as possible when describing medical conditions and medications. This will be considered confidential patient information and will be shared only with health care providers involved in the treatment of this potential patient. The progam is easy to use. Just choose one of the two methods below and follow these steps:

METHOD A. - Info INSIDE refrigerator:

  1. Complete a "Vial of Life" Medical Information Sheet. (pdf) Use the space on the back of the sheet for any additional information.
  2. Place the sheet in an sealed envelope marked "Vial of Life" and tape it to the inside of your refrigerator door.
  3. Then place a "Vial of Life" participant label (pdf) on the front of your refrigerator door.

METHOD B. - Info OUTSIDE refrigerator:

  1. Complete a "Vial of Life" Medical Information Sheet. (pdf) Use the space on the back of the sheet for any additional information.
  2. Place the sheet in an sealed envelope marked "Vial of Life" and tape it to the outside of your refrigerator door.
